Appendix K: HHS AI Use Case Tracker
The Department of Health and Human Services (HHS) reported 447 AI use cases in its FY2025 inventory, up from 271 in FY2024, but only 167 of the 447 were in deployed use; the rest were pilots, systems still in development or acquisition, or retired systems (OMB 2025 consolidated inventory; OMB 2024 consolidated inventory). This tracker keeps a dated, versioned copy of the HHS rows from both years, counts them by component, stage, and public health function, and lists every entry in a searchable table. Every entry is self-reported by the HHS component that runs it.

What the Inventory Does and Does Not Mean
Federal agencies inventory their AI use cases each year under Executive Order 13960 and the Advancing American AI Act, following reporting instructions from the Office of Management and Budget (OMB) (OMB 2025 reporting instructions; Executive Order 13960). HHS posts its inventory on its AI use case page (HHS AI Use Cases Inventory).
- It does mean an HHS component reported that it is developing, piloting, using, or has recently retired an AI system for the purpose stated in the entry.
- It does not mean the system works, has been evaluated, or improves health. The inventory records purpose and stage, not performance. Only high-impact use cases carry fields on testing, impact assessment, and monitoring, and the HHS rows in OMB’s consolidation leave those fields blank for every FY2025 entry, including the two designated high-impact.
- It does not mean the system is in wide use. A deployed entry can be a tool used by one office; the inventory does not report users, volume, or cost.
- It does not mean the list is complete. OMB’s 2025 instructions exclude AI used for basic or applied research and AI in national security systems, allow agencies to withhold use cases whose disclosure would conflict with law, and collect widely used commercial AI (for example chat assistants and meeting transcription) in a separate consolidated list that this tracker does not count (OMB 2025 reporting instructions).
- It does not mean HHS doubled its AI use. Reporting rules, stage definitions, and impact categories changed between the FY2024 and FY2025 inventories (see What Changed From FY2024 to FY2025).
An inventory entry is a disclosure, not evidence. Cite a published evaluation, not the inventory, when a claim depends on whether a system performs.
The Inventory in Numbers
The FY2025 inventory lists 447 HHS AI use cases and the FY2024 inventory lists 271 (OMB 2025 consolidated inventory; OMB 2024 consolidated inventory). Both counts include pre-deployment, pilot, and retired entries. Files retrieved October 30, 2026.

Component counts follow the bureau each entry names. Operating divisions are counted on their own; the Assistant Secretary for Financial Resources, the Office of the Chief Information Officer, the Office for Civil Rights, the Departmental Appeals Board, the Assistant Secretary for Technology Policy, the Office of the Assistant Secretary for Health, and other Office of the Secretary units are grouped together. FY2024 entries that named only “HHS” are shown as component not specified. ATSDR and NIOSH entries are reported under CDC.
What Changed From FY2024 to FY2025
The FY2025 inventory lists 176 more HHS use cases than FY2024, a 65% increase. CDC (55 to 103), FDA (27 to 67), CMS (41 to 73), and NIH (82 to 124) account for most of the rise. Three changes in the reporting rules limit what that difference means:
- Stage definitions changed. FY2024 used five life-cycle stages (initiated, acquisition and development, implementation and assessment, operation and maintenance, retired); FY2025 uses four (pre-deployment, pilot, deployed, retired). The mapped comparison above is the tracker’s approximation, not an OMB crosswalk.
- Impact categories changed. FY2024 asked whether a use case was rights-impacting or safety-impacting under OMB Memorandum M-24-10; FY2025 asks whether it is high-impact under M-25-21. HHS flagged 4 FY2024 entries and 2 FY2025 entries, but the two counts measure different definitions. One ACF entry, Structuring and Validating Completeness of Case Data Information, was safety-impacting in FY2024 and presumed high-impact but determined not high-impact in FY2025.
- Retired entries are reported once. OMB’s 2025 instructions require a retired use case to appear in one inventory after it stops operating, and allow agencies to remove prior entries that no longer meet the inclusion criteria. The 49 retired FY2025 entries are therefore a one-year record, not a cumulative count.
Names give a rough measure of continuity. Of the 447 FY2025 entries, 145 carry a name, or a reported former name, that matches an FY2024 entry after removing capitalization and punctuation. The other 302 are new, renamed without a reported former name, or split or merged from earlier entries; the inventories carry no stable identifier that would separate these cases.
Public Health Functions
HHS classifies each FY2025 entry by a broad topic area (for example Health and Medical, Admin Functions, or Science), which does not separate disease surveillance from grant administration. The tracker adds a public health function tag. A use case is tagged when its name, stated purpose, or stated outputs match one of eight published keyword rules:

The exact patterns are in the rules file. Each match was checked against the entry’s name and the text that triggered it. Where a keyword matched text that did not describe the system’s function (for example “VAERS” in a tool that sorts public docket comments), the tag was removed, and where a rule missed a clear case it was added; each such correction is listed with its reason in the overrides file. The tag is the tracker’s reading of the agency’s description, not an HHS classification, and entries outside the eight functions (such as clinical research, grant review, and internal assistants) are not tagged even when they support public health work indirectly.

The table lists every HHS entry in the FY2025 inventory; FY2024 entries are in the harmonized data file. The search box matches the name, bureau, and the start of the stated purpose. With scripts turned off, the full table is shown. Bureau, stage, topic area, and impact designation are as reported by HHS; the public health function column comes from the tracker’s rules.

HHS publishes its inventory as CSV files on its AI use case page, but hhs.gov refuses automated downloads of those files, and OMB’s sourcing notes for the 2025 consolidation record the same refusal (OMB 2025 consolidated inventory). The tracker therefore reads the HHS rows of OMB’s consolidated inventories on GitHub, which OMB compiles from each agency’s posted file: the FY2025 individually reported use cases and the FY2024 consolidated inventory (OMB 2024 consolidated inventory). Each file is pinned to a repository commit, and its SHA-256 checksum is stored in the tracker record. The HHS rows (447 for FY2025 and 271 for FY2024) are kept in the repository with their own checksums, line endings normalized.
Harmonizing. The two years use different column names and categories. The column crosswalk maps each tracker field to its FY2024 and FY2025 source columns and states which fields are comparable across years. Names are kept as reported, with whitespace collapsed; FDA names that read “Renamed: … Previously: …” are split into the current and former name. Each entry gets a tracker identifier built from its inventory year, bureau, and name, because the published inventories carry no stable identifier.
Refresh. A maintenance script (scripts/hhs_ai_use_case_tracker.py) downloads the pinned OMB files, verifies their checksums, extracts the HHS rows, and rebuilds the counts, table, and data files. A new inventory year is added as a new pinned source and a new tracker version; each refresh writes a new dated snapshot and leaves earlier snapshots unchanged.
Limitations
- Every entry is self-reported by the HHS component that runs it. The tracker does not verify that a system exists, is at the stated stage, or does what the entry describes.
- Counts include retired systems, pilots, and systems still in development or acquisition. The share of entries in deployed use differs by year and by component.
- The OMB consolidation is a copy of what HHS posted. If HHS revises its posted files after OMB’s consolidation, the two can differ until the next refresh.
- The public health function tag depends on how each entry describes itself. Brief or vague descriptions are under-tagged; the FY2025 retired entries carry no purpose text and are tagged only from their names.
- Year-over-year comparisons mix real change with changes in reporting rules, definitions, and agency practice.
